Crane Assembly and Disassembly Accidents: Causes, Liability, and Expert Analysis

OSHA Requirements for Assembly and Disassembly
OSHA's crane standard (29 CFR 1926, Subpart CC) includes specific requirements for assembly and disassembly operations in Section 1926.1404. Key requirements include:
Assembly/Disassembly Director
OSHA requires that a competent and qualified person direct assembly and disassembly operations. This person -- the A/D director -- must:
- Understand the applicable manufacturer procedures
- Have the ability to identify existing and predictable hazards
- Have the authority to take prompt corrective action
- Be present and direct all A/D operations
The A/D director is not optional. When an assembly or disassembly proceeds without a qualified director, or when the director fails to follow manufacturer procedures, the risk of a catastrophic failure increases substantially.
Manufacturer Procedures
OSHA requires that assembly and disassembly follow manufacturer procedures where available. When manufacturer procedures are not available or are insufficient, a qualified person must develop site-specific procedures. These procedures must address:
- The sequence of component installation or removal
- The capacity requirements for the assist crane
- Ground preparation and support requirements
- Pin and bolt installation torque specifications
- Structural inspection requirements before the crane is placed in service
Crew Qualifications
Every crew member involved in assembly or disassembly must be trained in the hazards associated with the operation and must understand their role. The assist crane operator must be qualified to operate the specific type of assist crane being used.
Common Causes of Assembly and Disassembly Accidents
1. Failure to Follow Manufacturer Procedures
Every crane manufacturer provides detailed assembly and disassembly procedures that specify the exact sequence of steps, the required tools and equipment, and the safety precautions that must be taken. When crews deviate from these procedures -- whether to save time, because they do not have the manual on site, or because they believe they know a better way -- the results can be catastrophic.
2. Inadequate Assist Crane Capacity
The assist crane must have sufficient capacity to safely handle the heaviest component at the required radius. When the assist crane is undersized or positioned incorrectly, it can become overloaded during the lift, leading to a tip-over of the assist crane or a dropped component.
3. Missing or Improperly Installed Pins
Lattice boom cranes, crawler cranes, and cranes with jib attachments rely on structural pins to connect boom sections, jib sections, and other components. A single missing or improperly seated pin can cause a structural failure when the crane is loaded. These failures often occur on the first lift after assembly, when the full structural load is applied for the first time.
4. Ground Failure During Assembly
During assembly, heavy components are being lifted and positioned while the crane and assist crane are on potentially unprepared ground. If the ground fails under either crane's outriggers or tracks during these operations, the consequences can be devastating. Ground conditions must be evaluated and prepared before assembly begins.
5. Wind and Weather
Large boom sections and jib components have significant surface area and are susceptible to wind loading during assembly. High winds can cause components to swing, shift, or twist while being positioned, making it impossible to align connection points safely. Most manufacturers specify maximum wind speeds for assembly operations.

- The Crane Owner/Rental Company -- responsible for providing a crane in proper condition with all necessary components, pins, and hardware. Also responsible for providing the manufacturer's assembly manual.
- The Assembly/Disassembly Director -- personally responsible for directing the operation safely and in accordance with manufacturer procedures or an equivalent site-specific plan.
- The Employer -- responsible for ensuring the A/D director is qualified, crew members are trained, and adequate equipment is provided.
- The General Contractor -- as the controlling entity, responsible for ensuring ground conditions are adequate and that the operation is being conducted safely.
- The Crane Manufacturer -- may be liable if the assembly procedures were inadequate, if components had manufacturing defects, or if the manual failed to warn of specific hazards.
